Fusion Form (フュージョンフォーム Fu~yūjon Fōmu) is the fusion and final upgrade of both Cure Starlight and Cure Dream. However, in the first transformation, Starlight did not fuse with Dream, due to Dream's unknown whereabouts. She instead used the power of the Mark of Hope, therefore giving her the Fusion Form. Her main attack is Crystal Fusion.

Mark of Hope[]
Cure Starlight takes out the Dream Tracer and lends her power to it, and her clothes disappear in a flash of crystals. She draws a circle of crystals and spins around in the circle. The crystals then explode, and Fusion Hope appears. She opens her eyes (which have beams of light in them) and says her introduction before striking her final pose.

- She is probably the strongest Pretty Cure yet, since none of the Pretty Cures could block the attacks from the enemies without the slightest effort.
- The other Cures can also transform into their own Fusion Forms, but due to their lights already there, it just takes a flash to transform, while Cure Starlight just needs the Dream Tracer due to the unknown whereabouts of her sister until PC5R44.
- Fusion Passion is the only fusion with weapons.
- Fusion Fragrant is the only fusion with wings.
- Each of the fusions have two voice actresses, and they are the voice actresses of the Cures who have fused.
